A
form will be in your child's school packet. Depending on your
school
the packet is sent out one of two ways:
- The
first day of school
- Over
the summer
If the student does not get the form, they can download
it, have the
parent fill it out and then return it to the school or to
myself if
that is the school we are currently at.

Out-Reach Services Available:
MDHOI
does not only provide services to schools. During the summer
vacation we will be more than happy to talk with other organizations
who could benefit from our services.
